Guardian agents: New approach could reduce AI hallucinations to below 1%

Hallucination is a risk that limits the real-world deployment of enterprise AI.

Many organizations have attempted to solve the challenge of hallucination reduction with various approaches, each with varying degrees of success. Among the many vendors that have been working for the last several years to reduce the risk is Vectara. The company got its start as an early pioneer in grounded retrieval, which is better known today by the acronym Retrieval Augmented Generation (RAG). An early promise of RAG was that it could help reduce hallucinations by sourcing information from provided content.

While RAG is helpful as a hallucination reduction approach, hallucinations still occur even with RAG. Among existing industry solutions most  solutions focus on detecting hallucinations or implementing preventative guardrails, Vectara has unveiled a fundamentally different approach: automatically identifying, explaining and correcting AI hallucinations through what it calls guardian agents inside of a new service called the Vectara Hallucination Corrector.

The guardian agents are functionally software components that monitor and take protective actions within AI workflows. Instead of just applying rules inside of an LLM, the promise of guardian agents is to apply corrective measures in an agentic AI approach that improves workflows. Vectara’s approach makes surgical corrections while preserving the overall content and providing detailed explanations of what was changed and why.

The approach appears to deliver meaningful results. According to Vectara, the system can reduce hallucination rates for smaller language models under 7 billion parameters, to less than 1%.

“As enterprises are implementing more agentic workflows, we all know that hallucinations are still an issue with LLMs and how that is going to exponentially amplify the negative impact of making mistakes in an agentic workflow is kind of scary for enterprises,” Eva Nahari, chief product officer at Vectara told VentureBeat in an exclusive interview. “So what we have set out as a continuation of our mission to build out trusted AI and enable the full potential of gen AI for enterprise… is this new track of releasing guardian agents.”

The enterprise AI hallucination detection landscape

Every enterprise wants to have accurate AI, that’s not a surprise. It’s also no surprise that there are many different options for reducing hallucinations.

RAG approaches help to reduce hallucinations by providing grounded responses from content but can still yield inaccurate results. One of the more interesting implementations of RAG is one from the Mayo Clinic  which uses a ‘reverse RAG‘ approach to limit hallucinations.

Improving data quality as well as how vector data embeddings are created is another approach to improving accuracy. Among the many vendors working on that approach is database vendor MongoDB which recently acquired advanced embedding and retrieval model vendor Voyage AI.

Guardrails, which are available from many vendors including Nvidia and AWS among others, help to detect risky outputs and can help with accuracy in some cases. IBM actually has a set of its Granite open-source models known as Granite Guardian that directly integrate guardrails as a series of fine-tuning instructions, to reduce risky outputs.

Using reasoning to validate output is another potential solution. AWS claims that its Bedrock Automated Reasoning approach catches 100% of hallucinations, though that claim is difficult to validate.

Startup Oumi offers another approach, validating claims made by AI on a sentence by sentence basis by validating source materials with an open-source technology called HallOumi.

How the guardian agent approach is different

While there is merit to all the other approaches to hallucination reduction, Vectara claims its approach is different.

Rather than just identifying if a hallucination is present and then either flagging or rejecting the content, the guardian agent approach actually corrects the issue. Nahari emphasized that the guardian agent takes action. 

“It’s not just a learning on something,” she said. “It’s taking an action on behalf of someone, and that makes it an agent.”

The technical mechanics of guardian agents

The guardian agent is a multi-stage pipeline rather than a single model.

Suleman Kazi, machine learning tech lead at Vectara told VentureBeat that the system comprises three key components: a generative model, a hallucination detection model and a hallucination correction model. This agentic workflow allows for dynamic guardrailing of AI applications, addressing a critical concern for enterprises hesitant to fully embrace generative AI technologies.

Rather than wholesale elimination of potentially problematic outputs, the system can make minimal, precise adjustments to specific terms or phrases. Here’s how it works:

  1. A primary LLM generates a response
  2. Vectara’s hallucination detection model (Hughes Hallucination Evaluation Model) identifies potential hallucinations
  3. If hallucinations are detected above a certain threshold, the correction agent activates
  4. The correction agent makes minimal, precise changes to fix inaccuracies while preserving the rest of the content
  5. The system provides detailed explanations of what was hallucinated and why

Why nuance matters for hallucination detection

The nuanced correction capabilities are critically important. Understanding the context of the query and source materials can make the difference between an answer being accurate or being a hallucination.

When discussing the nuances of hallucination correction, Kazi provided a specific example to illustrate why blanket hallucination correction isn’t always appropriate. He described a scenario where an AI is processing a science fiction book that describes the sky as red, instead of the typical blue. In this context, a rigid hallucination correction system might automatically “correct” the red sky to blue, which would be incorrect for the creative context of a science fiction narrative. 

The example was used to demonstrate that hallucination correction needs contextual understanding. Not every deviation from expected information is a true hallucination – some are intentional creative choices or domain-specific descriptions. This highlights the complexity of developing an AI system that can distinguish between genuine errors and purposeful variations in language and description.

Alongside its guardian agent, Vectara is releasing HCMBench, an open-source evaluation toolkit for hallucination correction models.

This benchmark provides standardized ways to evaluate how well different approaches correct hallucinations. The goal of the benchmark is to help the community at large, as well as to help enable enterprises to evaluate hallucination correction claims accuracy, including those from Vectara. The toolkit supports multiple metrics including HHEM, Minicheck, AXCEL and FACTSJudge, providing comprehensive evaluation of hallucination correction effectiveness.

“If the community at large wants to develop their own correction models, they can use that benchmark as an evaluation data set to improve their models,” Kazi said.

What this means for enterprises

For enterprises navigating the risks of AI hallucinations, Vectara’s approach represents a significant shift in strategy. 

Instead of just implementing detection systems or abandoning AI in high-risk use cases, companies can now consider a middle path: implementing correction capabilities. The guardian agent approach also aligns with the trend toward more complex, multi-step AI workflows.

Enterprises looking to implement these approaches should consider:

  1. Evaluating where hallucination risks are most critical in their AI implementations.
  2. Considering guardian agents for high-value, high-risk workflows where accuracy is paramount.
  3. Maintaining human oversight capabilities alongside automated correction.
  4. Leveraging benchmarks like HCMBench to evaluate hallucination correction capabilities.

With hallucination correction technologies maturing, enterprises may soon be able to deploy AI in previously restricted use cases while maintaining the accuracy standards required for critical business operations.

Crude Rebounds Over 11% from Lows

Trump DOE’s latest move would roll some energy standards back decades

Dive Brief: The U.S. Department of Energy moved Monday to repeal or alter 47 energy efficiency, climate reporting and other regulations, but experts say the Trump administration will likely face robust legal challenges. Intel Certifies Shell Lubricant for Cooling AI Data Centers

Intel Corp. has certified Shell Plc’s lubricant-based method for cooling servers more efficiently within data centers used for artificial intelligence. The announcement on Tuesday, which follows the chipmaker’s two-year trial of the technology, offers a way to use less energy at artificial intelligence facilities, which are booming and are expected to double their electricity demand globally by 2030, consuming as much power then as all of Japan today, according to the International Energy Agency. So far, companies have largely used giant fans to reduce temperatures inside AI data centers, which generate more heat in order to run at a higher power. Increasingly, these fans consume electricity at a rate that rivals the computers themselves, something the facilities’ operators would prefer to avoid, Intel Principal Engineer Samantha Yates said in an interview. “Upgrading existing air-cooling methods with immersion fluids can reduce data center energy use by up to 48%, as well as help reduce capital and operating expenditure by up to 33%,” Global Executive Vice President of Shell Lubricants, Jason Wong, said in a written statement. The immersion cooling fluids are ready to deploy and Intel is “providing an immersion rider warranty on top of our standard warranty terms to say we believe in this so much that you will be successful,” Yates said. Shell’s technology is the first of its kind to receive official certification by a major chip manufacturer, the companies said. GOP to Phase Out Biden Energy Credits to Pay for Tax Cuts

House Republicans are proposing to eliminate a tax credit for electric vehicles and phase out incentives to develop clean-energy projects to help pay for President Donald Trump’s massive tax package. The incentives put in place by former President Joe Biden’s signature climate law have been ripe targets for lawmakers looking for trillions of dollars to help pay for extending Trump’s tax cuts. The president himself has had a bullseye on them, deriding them as part of the “green new scam.” But the draft legislation released Monday by House tax writers may not be as bad for producers of clean electricity from sources such as solar and wind, who feared a more aggressive phase out. First Solar Inc., the largest US solar manufacturer, rose 11% on Monday. Sunrun Inc., the largest US residential solar company, rose nearly 17%. “The proposal is mostly a win for US solar manufacturers and developers,” said Rob Barnett, senior analyst at Bloomberg Intelligence. “The fear is that the investment and production tax credits could have been gutted sooner.” In the Republicans’ proposal, popular production and investment tax credits for clean electricity would be phased out by the end of 2031 and new requirements against using materials from certain foreign nations would be added. House GOP proposes early phaseout of IRA clean energy tax credits

Dive Brief: Federal tax credits that benefit energy developers, manufacturers and utilities face an early phaseout in a budget proposal released Monday by a key GOP-controlled House committee. The House Ways and Means Committee’s draft reconciliation package steps down the investment and production tax credits for nuclear power, wind, solar, batteries, geothermal and other clean energy technologies after 2028, and eliminates them completely after 2031. It preserves a comparatively generous credit for carbon sequestration and extends the clean fuels production credit. Energy industry groups and customers slammed the proposal, saying it would raise electricity prices, quash a manufacturing boom spurred by the Inflation Reduction Act and erode the United States’ competitive advantage on artificial intelligence. Dive Insight: The Ways and Means budget gives clean energy developers and producers until 2028 to claim the full 45Y and 48E tax credits for clean energy investment and production. Cisco joins AI infrastructure alliance

Cato Networks introduces AI-powered policy analysis engine

Cato Networks this week announced a new policy analysis engine for its cloud-based secure access service edge platform that the company says will optimize and improve SASE policies, reduce risk, simplify compliance, and reduce manual maintenance efforts. Cato Autonomous Policies is built into the Cato SASE Cloud Platform and can provide enterprises with AI-driven recommendations to eliminate security exposure, tighten access controls, and improve network performance. The first use case of the policy engine is designed for firewall as a service (FWaaS) environments in which “firewall rule bloat” is present, Cato explained in a statement. The bloat comes from organizations accumulating thousands of rules that were designed to protect the environment, but after becoming outdated or misconfigured, actually lead to increased risk. “Most enterprises rely on a mix of firewalls deployed in data centers, branch offices, and cloud environments. Over time, rule sets grow, become inconsistent, and are filled with redundant, outdated, or conflicting entries,” wrote Demetris Booth, product marketing director at Cato Networks, in a blog post on the product news. “As a result, security policies become hard to manage, even harder to audit, and often misaligned with zero-trust principles. AI-driven firewall policy management is necessary for modern enterprises to streamline and optimize security operations.”

Riverbed bolsters network acceleration for AI’s performance bottlenecks

Riverbed SteelHead Virtual, is a software-only version designed for virtualization environments and private cloud deployments, which is compatible with VMWare ESXI, KVM, and Microsoft Hyper-V. “For customers that are familiar with Riverbed, this is a big change in performance. We’ve gone from moving one appliance at 30 Gbps to 60 Gbps. Kyndryl and Microsoft expand partnership to streamline cloud operations

Kyndryl and Microsoft broadened their existing partnership to bring together Microsoft’s adaptive cloud approach and Kyndryl Distributed Cloud services to help customers better develop, manage and secure hybrid cloud operations.  Microsoft says its AI-infused adaptive cloud approach, which leverages Microsoft Azure Arc, Azure Local and Azure Cloud, enables customers to link distributed hybrid, multicloud, edge, and IoT resources under a single, secure application and data platform. The model uses customer data and an AI engine to offer predictive analytics, automated workflows, and threat detection and response to manage the environment, according to Microsoft.  Kyndryl will deliver the adaptive cloud approach to its customers through its Distributed Cloud services, which also use AI to improve automation, optimize workloads, enhance application performance, and reduce operational complexity. Kyndryl Distributed Cloud services create a mesh of interconnected resources and data from the data center to the edge in a multicloud environment, according to Kyndryl. Tech CEOs warn Senate: Outdated US power grid threatens AI ambitions

Microsoft will invest $80B in AI data centers in fiscal 2025

And Microsoft isn’t the only one that is ramping up its investments into AI-enabled data centers. Rival cloud service providers are all investing in either upgrading or opening new data centers to capture a larger chunk of business from developers and users of large language models (LLMs).  In a report published in October 2024, Bloomberg Intelligence estimated that demand for generative AI would push Microsoft, AWS, Google, Oracle, Meta, and Apple would between them devote $200 billion to capex in 2025, up from $110 billion in 2023. Microsoft is one of the biggest spenders, followed closely by Google and AWS, Bloomberg Intelligence said. Its estimate of Microsoft’s capital spending on AI, at $62.4 billion for calendar 2025, is lower than Smith’s claim that the company will invest $80 billion in the fiscal year to June 30, 2025. Both figures, though, are way higher than Microsoft’s 2020 capital expenditure of “just” $17.6 billion. The majority of the increased spending is tied to cloud services and the expansion of AI infrastructure needed to provide compute capacity for OpenAI workloads. John Deere has revealed a new line of autonomous machines and tech across agriculture, construction and commercial landscaping. The Moline, Illinois-based John Deere has been in business for 187 years, yet it’s been a regular as a non-tech company showing off technology at the big tech trade show in Las Vegas and is back at CES 2025 with more autonomous tractors and other vehicles. This is not something we usually cover, but John Deere has a lot of data that is interesting in the big picture of tech. The message from the company is that there aren’t enough skilled farm laborers to do the work that its customers need. It’s been a challenge for most of the last two decades, said Jahmy Hindman, CTO at John Deere, in a briefing. Much of the tech will come this fall and after that. He noted that the average farmer in the U.S. is over 58 and works 12 to 18 hours a day to grow food for us. And he said the American Farm Bureau Federation estimates there are roughly 2.4 million farm jobs that need to be filled annually; and the agricultural work force continues to shrink.
